Air India Flight AI171 crashed on June 12, killing 260 people. The preliminary report by the AAIB suggests that the engine fuel switches were flipped to CUTOFF just seconds after takeoff, causing a loss of thrust. The pilots were unaware of this and tried to recover control of the aircraft before impact.
